What is Intensive Outpatient?

An Intensive Outpatient Program (IOP) is a structured, clinically supervised level of behavioral health treatment designed for individuals who need more support than traditional weekly counseling but do not require 24-hour residential or inpatient care.

For substance use disorders, mental health disorders, or co-occurring disorders, an IOP allows people to receive intensive treatment while continuing to live at home, work, attend school, or participate in a recovery residence such as Hope is Alive.

What is the purpose of an IOP?

The goal of an IOP is to help individuals:

  • Stop using alcohol or drugs and maintain sobriety.
  • Develop healthy coping skills.
  • Learn to manage cravings and triggers.
  • Treat underlying mental health conditions such as depression, anxiety, PTSD, or bipolar disorder.
  • Improve relationships and communication.
  • Prevent relapse.
  • Build a healthy recovery lifestyle.
  • Transition successfully from inpatient treatment or avoid the need for hospitalization.

Who should attend an IOP?

An IOP is appropriate for individuals who:

  • Have a substance use disorder that requires structured treatment.
  • Have both substance use and mental health disorders (co-occurring disorders).
  • Recently completed detox or residential treatment.
  • Have experienced relapse and need additional support.
  • Need more than weekly therapy but do not require 24-hour supervision.
  • Are motivated to participate in treatment several days per week.
  • Have a stable and safe living environment or are living in a structured recovery residence.

What happens during an IOP?

While every program is different, most IOPs include:

  • Group therapy (the primary component)
  • Individual counseling
  • Psycho education
  • Relapse prevention training
  • Cognitive Behavioral Therapy (CBT)
  • Dialectical Behavior Therapy (DBT) skills
  • Family education or family therapy
  • Case management
  • Recovery planning
  • Drug and alcohol screening when appropriate
  • Medication management or psychiatric services (if offered or referred)

How many hours per week is IOP?

  • 3–5 days per week
  • Approximately 3 hours per session
  • 9–20 hours of treatment per week

How long does the program last?

Programs generally last 6–12 weeks, although the length depends on each person's clinical needs and progress.
