Schedule a confidential 20-minute phone call with a Life Change Outpatient staff member. During the screening, we'll gather your insurance information to verify benefits and complete a brief assessment to help us prepare for your intake appointment. Please have your insurance card ready. All information shared is kept confidential and used solely for treatment planning and verification.
